your thread title  contradicts that theory .... Space programs are risky and anything can happen .

 

All three stages were normal except the last stage ,where heat shield couldn't separate itself from the satellite  .I'm sure they will be successful and fix the issue before the launch of GSLV in October /November

36 launches in 20 yrs isn't a big number and that was with ISRO was handholding the suppliers all the way through the process. As they are looking to up the launch number to 12+ a year from now, Suppliers would have to take on more responsibility - which they have not shown to be capable of. Consequently, 12+ launches goal, will be postponed. 

DRDO knows this first hand (since the production numbers tend to be couple of order of magnitude greater). Nirbhay cruise missile development is facing delays because in the last test (which failed) they found out that recycled steel was used, so forget about manufacturing QC, one can't even trust these people with using the right materials.
