The 13th match possibility is if Bangladesh beat Australia and Pakistan get eliminated, then Australia will take Pakistan's place and go to the other group. The India- SL game is unlikely. If SL qualify they won't be in India's group.

Its not that simple. New Zealand and West Indies come to the group with 2 wins each. Pakistan and Safrica have one each. Pakistan and South Africa have to win at least two of their three games and even them they may have to fight it out for run rate. Its not that easy. To be hundred % sure Pakistan needs to win all the games.

What happened before the super 8s is immaterial. This is now a clean slate start. The top 2 teams in the Super 8 stages progress. It is as simple as that.
