His captaincy cost SA the match (not his batting or bowling). Some of his glaring errors: -Replacing the in-form Abbott with Philander -Bowling part-timers just after the 4th wicket fell instead of going for the kill -Bowling himself in the death overs and proving expensive -Asking Steyn to bowl a length ball in the last ball of the match hoping that Elliot would play and miss. IMO an accurate yorker or fast bouncer was needed that ball.

1 & 3 are not his mistakes...Philander is probably mandatory quota. He bowled because there is no other option [5th bowler Duminy was going 8+ an over] 4 also not sure whether he asked, normally it will be bowlers or collective decision.

Yes, he had a discussion with the bowler and they decided on a length ball. It was both their fault. Steyn is perfectly capable of bowling accurate yorkers.
