Just keep the whistle/pores clean and not overload the cooker. And if still things go wrong, you have the safety valve - which is what I guess exploded in Monisha's case.

He ment the rubber might've gone hard and not flexible enough to give proper sealing.

She said there was a loud noise which could be due to steam and content coming out through some gap and spraying all over..

That sort of explosion is rare and dangerous, I am talking about valve-melt-exploding only. Happened to me couple of years back.
