Its like the case with FDR - he had polio so the world media never ever (with the exception of just ONE picture) showed the lower half of his body in pictures they took of him. It was an informal rule among the media - something which didn't need to be ordered or told by anyone to follow.

Sometimes I think that MMS saw AtalJi as the leader he himself could never be, and being the gentleman that he is, instead of being jealous he became an admirer. Even when he was PM he ALWAYS went to AtalJi's house on his birthday or when he became too ill from time to time. Also, AtalJi is respected by everyone -be it his own party members or opposition members. Not a single party or leader raised any questions or opposed him getting Bharat Ratna.
