Bhuvi, McGrath, Kumble, Akhtar- all put the same amounts of effort while delivering the ball. The difference in speed between these guys is not a factor of effort, its a factor of technique. The technique you are talking about just helps you direct your efforts in a right way and not waste them and it is a fact the slower you are, the more chance you have to maintain your speeds and no 135 was not McGrath's top speed. In his early days, he could crank up to over 140s and even when he slowed down, it is not like he would maintain it whole day. He would even go below 130K sometimes. Look, the average speed will vary a lot depending on how much they are using the slower ball. But the stock deliveries of McGrath, like Marshall and Holding, stayed at 95% top speed for pretty much their whole spells. That is because they were fit as fit can get. And then you cannot compare humans to deer. Deer can maintain because they are made to do that, but not us. Does not matter how great a stamina you have, you will struggle to maintain your pace throughout the day bowling 25 overs in a test. Not if bowling at 95mph comes as naturally to you as bowling at 135kph came to McGrath. You will struggle to bowl at 95mph if 95mph is a product of 'over-comitting' to bowling fast, sort of being the guy who huffs and puffs to get that extra 1mph out instead of bowling at their natural pace. When you are talking about fitness freaks like McGrath, Walsh, Marshall and Holding, they could bowl at their natural paces all day because they were capable of running 10km every morning before breakfast and were far far fitter than Akhtar or Lee. its your fitness that determines how long you can sustain your pace. If you are Marshall, you could bowl at 150kph all day because he was fit enough to bowl at his natural pace all day long. That is what makes Marshall and Holding a far more threatening option than Lee or Akhtar.
