People used to smoke in different ways and had different names for it. Also cigarettes too have some kind of tobacco inside them AFAIK.

Because deep down everyone knows that England is the UK In fact the only other people from UK nations that would bother waving the Union Jack outside of England are the Northern Irish Protestants and that is just to get back at the Irish. There is a general injunction that prohibits one from indulging in activities which harm one's self. So, it is open to interpretation. There have recent fatwas against the (mal)habit. Most of my Muslim friends had no issues smoking up , but dint drink . Never got the logic .

It's probably a cultural thing that has somehow evolved from the religion but makes no sense. It's the same with smoking for Sikhs. Most will not do it and if they do if will be done very discreetly. However there is no problem with drinking copious amounts of alcohol even though the religion strictly forbids it.
