The ball literally twists in the sack which causes the spermatic cord to twist and and cut off blood supply to the balls. You literally have just 4-5 hours to save the balls and only with surgery. After that time one or both of your balls are gone forever and needs to be cut off. The pain is also worse than kidney stones and you vomit and feel nauseous with the pain. Can happen without any reason, like during sleeping or too much growth spurt during puberty or even injury like a ball hitting your balls while playing. Happened to a friend of mine.
